Support Coordination with NDIS

Our support coordinators help you plan ahead for plan review by NDIS. We will optimise your plan to get the most out of the funded support.

Aged Care Services

Our compassionate aged care team supports older adults and seniors to stay safe, independent, and comfortable at home. We can assist with personal care, domestic support, transport, social support, respite, and nursing care, with support tailored to each person’s needs.

Supported Independent Living

We specialise in Supported Independent Living (SIL), a service to provide a safe, stable, and secure home where individuals with additional needs can reach their full potential and live as independently as possible. Housing options in Spring Farm, Camden, Leumeah, Campbelltown Werombi & Macarthur Heights NSW.

Therapeutic Care

We have allied health professionals to help you overcome your specific condition with a variety of therapy services, counselling, or psychology.

Comprehensive Support

The Correlate Connections support team provides assistance for a range of areas including cooking, shopping, personal care, paying bills, etc.

Childrens Services

We specialise in After School Care and Vacation Care and also provide Boys Club Programs and Girls Club Programs –  a fun and social indoor and outdoor adolescence service focusing on independence and life skills.

Adult Support Services

Correlate Connections provide Adult Disability Day Programs and Group Programs for our adult clients in need.
